Terms & Conditions for my Shopko rewards® extra savings Program ("my extra savings Program") Details

Program Details

Participants must be a member in the my extra savings program to participate. To become a member you must complete a registration form either at a Shopko store on on-line at www.shopko.com. My extra savings program participation and its benefits are offered at the discretion of Shopko. Shopko has the right in its sole and absolute discretion to modify, add or delete any of the my extra savings program rules, terms, conditions, benefits or rewards, in whole or in part, at any time, with or without notice.

How It Works

Present your my extra savings membership card at checkout to receive special savings offers. You must present your extra savings card with every purchase in order to take advantage of these special offers. If you do not have your my extra savings program card with you, Shopko cashiers will be able to look up your membership number through the use of your home phone number. No my extra savings program price adjustments will be given after the transaction has been completed. Shopko reserves the right to make my Shopko my extra savings promotional offers available to select members based on purchase activity, geographic location, my extra savings program participation or information supplied by the member. Some of the offers Shopko elects to include in the my extra savings program may be subject to additional terms and conditions

Extra 10% off for your Birthday

When you make a purchase and present my extra savings card within 14 days before or after your birthday you will receive a cash register receipt that acknowledges your birthday and entitles you to 10% off your next purchase. Discount must be used within 14 days of receipt. Discount must be taken at register. Must be used with Shopko cash register receipt that includes "birthday message" and presented at the time or purchase. If you provided an email address you will receive you 10% birthday offer in an email before your birthday month. Your email birthday offer must be used within the month of your birthday. Either offer cannot be combined, copied, transferred or sold. Not valid on previous purchases, online at SHOPKO.COM or redeemable for cash. Discount does not apply to prescriptions, diabetic supplies, pharmacy special-order items, doctor's fees, eye exams, Shopko gift cards, digital music or retail gift cards, disposable baby diapers, baby formula & alcoholic beverages. Rainchecks issued on day of event will not include discount when redeemed.

Ten for $10 Club

See store for details. Coupon received when 10th prescription is purchased. Coupon good on your $20 purchase of regular, sale & clearance priced merchandise before tax. Not valid for online purchases. No adjustments on prior purchases. Coupon and my extra savings card issued to the cardholder who accumulated the points must accompany purchase. Coupon cannot be copied or transferred. Coupon can only be redeemed for merchandise, it cannot be redeemed for cash. Club participation is limited to customers paying cash or insurance programs not funded in whole or in part by federal or state subsidized programs. Purchase excludes prescriptions, diabetic supplies, pharmacy special-order items, doctor's fees, eye exams, mycontacts.shopko.com, Shopko gift cards, digital music cards, video gaming cards, retail gift cards, prepaid bank cards, disposable baby diapers & wipes, baby formula, alcoholic beverages and tobacco. Rainchecks issued on day of event will not include discount when redeemed. For every returned item, a portion of the discount will be applied to the refunded amount. Coupon is good for 30 days from date issued.

Eligibility

Participants must register on an individual basis and be at least 18 years of age. Customers purchasing items for resale are not eligible for my extra savings Program savings. Employees of Shopko and their immediate family are eligible to participate, but may not combine my extra savings pricing with employee discount. Participation in the my extra savings Program is void if: a) prohibited by law in the jurisdiction in which the prospective participant resides or b) a special license, bonding, permit or other governmental regulatory approval is necessary in that particular state. In the event of any discrepancies regarding membership, contact OnlineCustomerService@shopko.com or call the Shopko Customer Service number (1-800-791-7333).

Member Responsibilities

You may not permit others access/use of the card. In the event a membership card is misplaced, lost or stolen, immediately contact OnlineCustomerService@shopko.com or call the Shopko Customer Service number (1-800-791-7333). You are responsible for notifying Shopko of any changes to your personal information including name, address, and other contact information.

Term of Membership

This Program will continue until such time as Shopko, in it's sole discretion, selects to end the Program. Shopko has the right to end the Program at any time without providing written notice to you. You may cancel your membership in the Program at any time by notifying Shopko Customer Service by mail, e-mail or telephone at the address found below. We will cancel your membership in the Program if you fail to use the my extra savings membership card for eighteen (18) months or fail to comply with these Terms and Conditions.

Privacy Policy

Shopko respects your privacy. We maintain safeguards to protect your personal information. By signing up to participate in the my extra savings Program, you agree that Shopko may use all information acquired by Shopko as a result of your participation in the my extra savings Program. Shopko may use such information for account management, customer service and related purposes and otherwise as permitted or required by law. You authorize Shopko and/or our marketing partners to use such information to send you marketing communication and special offers either by mail, email or mobile device. Participants that do not consent to such use of their contact information can either email OnlineCustomerService@shopko.com or call the Shopko Customer Service number (1-800-791-7333) to unsubscribe.

Legal Notices Modification of these terms and conditions may only be made in writing by an authorized representative of Shopko. All interpretations of my extra savings Program Terms and Conditions shall be at the sole discretion of Shopko and such interpretations are binding on you.

For purposes of my extra savings Program Terms and Conditions, "Shopko Stores Operating Co., LLC" shall include Shopko Express, as applicable. The Shopko logo, my extra savings logo and other logos and tag lines are the intellectual property of Shopko Stores Operating Co., LLC.
